Describe the differences between treatments at inpatient versus outpatient facilities.

Inpatient is a higher level of care than outpatient. Outpatient level of care (Level I) typically consists of meeting with a therapist once a week for 45 minutes to one hour, or twice a week if needed. Outpatient level of care is accessible to most at it is the lowest level of care in therapy (Capuzzi & Stauffer, 2016). The client is expected to be able to be regulated enough after the session to return home. Clients identify triggers, discomforts, and tools to replace the addictive behaviors (and related risk behaviors).

Some outpatient facilities and providers have their own on-call emergency 24 hour clinician available in case of a non-medical emergency. Others have either this service, and/or refer out to local hospital psychiatric emergency rooms, or mobile crisis services. Clients in substance use outpatient treatment often attend group and family therapy as well. They are at lower risk than those at an inpatient facility. Some however due engage in Level II treatment, or intensive outpatient treatment (Capuzzi & Stauffer, 2016). Its intensity is demonstrated in increased frequency and wrap around services encouraged. Medical interventions might also be included with a medical provider at the agency, or the client is referred out. It is helpful to work collaboratively with other providers involved in the treatment.

Inpatient is a locked facility where clients are to be monitored 24/7. Clients are admitted into inpatient when they are at risk to themselves or others. An inpatient facility is stricter due to the need to closely monitor clients. Family and social supports visit and engage in treatment in efforts of working towards discharge (Capuzzi & Stauffer, 2016). Before placement, clinicians process “placement matching” to ensure the client receives adequate, specific treatment to them.

What are some of the clinical treatment strategies, including individual, group, and family methods?

Assessing for level of risk prior to and during admission is essential to ensure the client is in the most suitable level of care for them. One method used by clinicians is the Multidimensional Risk Profile (Capuzzi & Stauffer). The MRP considers the clients status throughout treatment, and the risk of recidivism. Educating the client and the family that relapse is part of the recovery is essential. This provides a non-judgmental approach. Seeing clients as dimensional beings also respects the way in which dimensions are interrelated, increasing awareness of the challenges a client may encounter. A multidimensional disease requires a multidimensional approach. While families are moving through their own healing process, they are also supported in preparing the home for recovery. Families attend sessions to learn about their relationship or role, to the loved one’s addiction. This includes building healthy communication and setting

Clients will experience differences in their treatment based on whether they are involved in inpatient or outpatient facility, and for inpatients, on whether their care is administered in a private or public sector. A majority of public inpatient programs accommodate the underinsured, uninsured, or indigent and most funding is underwritten from local or federal assistance programs. A program like this is typically 28 days long, but can run longer if detoxification is needed and shorter (like 10 days) once a patient is stabilized. Public programs are generic and not specific to individualized needs and services may include managed detoxification, substance abuse psychoeducation classes, and individual and group sessions. In a private inpatient setting, a client is admitted to a facility and their stay generally lasts 30 to 90 days. They are encouraged to bring personal belongings and pictures of loved ones. During the active phase, clients participate in psychoeducation, sleeping is regimented and relationships are not allowed (Capuzzi, & Stauffer, 2016). In an outpatient setting, individuals will attend treatment in an office or hospital and then leave. They visit their family members, engage in activities and attend work. Aside from being admitted to the treatment facility, an individual involved in an outpatient regimen includes similar levels of treatment and treatment strategies as an individual admitted to an inpatient program (Inpatient vs. Outpatient Treatment for Alcohol Addiction, 2021).

Clinical treatment strategies consist of individual therapeutic techniques like Cognitive Behavioral Therapy, Contingency Management Therapy, Dialectal Behavioral Therapy and medication; group therapy like 12-Step Facilitation therapy and Experiential therapy; and family therapy like the Matrix Model (Thomas, 2021).

12-step programs are self-help groups that can compliment the effects of professional treatment, and groups like Alcoholics Anonymous (AA), Narcotics Anonymous (NA), and Cocaine Anonymous (CA) add a layer of community level support that is helpful to clients in maintaining healthy behaviors after formal treatment (Where Do 12-Step or Self-Help Programs Fit Into Drug Addiction Treatment?, 2021).
