Patient-Centered Medical Home (PCMH)
This is a medical care model that considers the needs and interests of the patients,
prioritizing patients’ needs for better healthcare outcomes. The model ensures that there is a good
relationship between the patient and the care providers which also ensures that there is improved
patient experience through health quality while ensuring that the healthcare providers are
satisfied (Perez Jolles et al., 2019). This outline highlights the origin and structure of patientcentered medical home, how the model reduces costs and improves quality, and the contracts that
dictate payment methods of healthcare providers.
Origin and Structure Patient-centered Medical Home
The origin of this care model dates back in history when healthcare providers intended to
provide healthcare services to children with chronic illnesses. The concepts evolved as
researchers were searching for high-quality services at a reduced cost. The healthcare model was
developed in the United States and was built under the concept of community and family
medicine (Perez Jolles et al., 2019). Under the model, the healthcare services are under the
primary physician who oversees the delivery of healthcare services by a team under their
supervision. The patient is under the care of an assigned physician who ensures that there is
continuity in quality care, addressing social, psychological, and medical needs of the patient. The
model also ensures that the needs of the patient are addressed effectively to ensure effective
health outcomes. The team may also involve additional care providers to ensure the interests of
the patient are met. The model promotes the development of long-term relationships between
patients and care providers and provides a holistic care approach addressing the needs of the
patient.

How PCMH contain the cost and improve care quality
The model reduces the cost of care delivery by eliminating unnecessary services offered
in the hospital setting. Based on research, patient-centered medical homes offer care services to
patients that are enhanced by well-coordinated teams that ensure that the patient receives the best
care at an affordable cost (Veet et al., 2020). There are reduced emergency visits, hospital readmission, inpatient hospitalization, and other unnecessary healthcare services. Given that the
model considers the interests of the patient such as unique culture, preferences, needs, and
values, the quality of healthcare is improved, ensuring that the patient is well taken care of. It
also ensures that there is effective decision-making and collaboration between caregivers,
patients, healthcare providers, and family members hence taking the interests of the patient and
family members at heart (Mold & Duffy, 2022). Involving all the care providers and ensuring
personalized care promotes quality care to the patient. The healthcare services offered are also
well coordinated by the lead physicians to ensure the safety and quality of healthcare service for
effective outcomes.
Health Care Providers’ Contracts and Payment Methods
The model utilizes contracts and payment methods dictated by the agreement between the
care providers, physicians, nurses, and other involved specialties. The contract may be drafted
for a given period after which it may be terminated or renewed based on how well the care
providers performed (O’Dell, 2016). There is always a negotiation between the care providers,
the lead physician, and the legal team to ensure a clear definition of all the components and
payment models. Payment methods are based on basic salary for the services offered, reward
based on the performance of the care providers, or paying healthcare incentives after achieving

given quality metrics or improved care delivery service that ensures patients’ outcomes are
improved significantly.
